To register you need to live within our catchment area


Catchment Area

If you are registered in our inner practice area and then move to our outer practice area you may remain on our list. Please enquire at reception if your new post code is in our outer boundary.

You can register:

  • By visiting the surgery in person and complete the application forms at reception.
  • Online through the GP Registration Service

If you are registering at the surgery you may find it quicker and more convenient to print off the forms required, complete them at home and bring them with you.

You should also bring the following:

  • 2 forms of identification for each person, one of which should be photo ID and one proof of address (eg utility bill).
  • An existing Health Card (with NHS number) if you have one (Find your NHS Number here)
  • Details of any existing Repeat Prescription medication you have from your previous surgery

New patients will normally be offered a preliminary health check when they register. All patients are allocated a named, accountable GP.

OUT OF AREA PATIENTS

We are not currently participating in this scheme as NHS England has not provided a service for home visits for out of area patients

TEMPORARY PATIENTS

If you are in the area for a short time you can register as a temporary resident

  • You can register temporarily with a GP while away from home for work, study or holiday. You’ll still remain registered with your permanent GP.
  • You can register as a temporary resident if you plan to live near the GP surgery for up to three months. After three months you will have to apply to register with that surgery as a permanent resident.
  • Your temporary GP will pass details of any treatment you have to your GP, who will add the information to your medical record.
